Warnings from the UK Drug Safety Watchdog

The Medicines and Healthcare products Regulatory Agency (MHRA) has issued warnings regarding the use of finasteride, a daily pill used to treat male pattern baldness. The agency has highlighted the potential for serious side effects, including sexual dysfunction and mood disorders.

Dutasteride, a similar drug used for treating symptoms of an enlarged prostate, has also been flagged with precautionary warnings. In 2024, the MHRA mandated that suppliers of finasteride include a patient alert card in drug packs to raise awareness of these risks.

These cards outline the potential risks such as erectile dysfunction, loss of libido, depression, and suicidal thoughts. Patients are advised to stop the drug immediately if they experience any of these symptoms. Those taking the drug for an enlarged prostate should contact their doctor urgently if they develop any adverse effects.

Dr Alison Cave, chief safety officer, emphasized the importance of updated product information for both finasteride 1mg and dutasteride. This ensures clearer guidance for healthcare professionals and patients, supporting informed discussions about treatment options.

How Finasteride Works

Finasteride, sold under brand names like Proscar and Propecia, works by blocking the conversion of testosterone into dihydrotestosterone (DHT). This hormone can contribute to prostate growth and hair loss. While it is available on the NHS for treating an enlarged prostate, it is only available through private prescriptions for male pattern baldness.

An enlarged prostate is a common condition among men over 50, typically not serious but can cause difficulty urinating due to pressure on the urethra. Symptoms may include a weak urine flow, longer time to empty the bladder, and increased urgency, especially at night.

Reviewing Evidence for Patient Safety

The MHRA has reviewed all available evidence to ensure patient safety. Dr Cave stated that patient safety remains a top priority. Prescribers are expected to discuss relevant safety information with patients, enabling them to make informed decisions about their treatment.

The Yellow Card scheme allows patients to report adverse effects of medicines. Since its inception, the scheme has received 170 reports related to suicidal ideation for finasteride, with 19 being fatal. Experts have concluded that stopping the drug does not always eliminate side effects, as some may persist even after treatment is discontinued.

Previously, the MHRA encouraged patients to share the alert card with friends and family, who may better notice changes in mood. However, accurate prescription numbers for finasteride 1mg are not currently available, as it is only available through private prescriptions.

Prescribing data indicates that more than 400,000 prescriptions per month are issued for finasteride 5mg and dutasteride containing medicines. This highlights the widespread use of these drugs and the ongoing need for vigilance regarding their potential side effects.
