Concerns Over Trump’s Mental and Physical Health
Many Americans continue to express concerns about the mental state of Donald Trump, with some drawing comparisons to the public perception of Joe Biden during his presidency. These worries have been amplified by recent events where the president’s speeches and rallies have increasingly featured tangents and seemingly irrelevant topics, which he refers to as “The Weave.” This has led to speculation about his cognitive abilities, especially as he prepares for a scheduled annual physical at Walter Reed Medical Center.
While it remains uncertain whether Trump will undergo one of the “cognitive tests” he often boasts about acing, the results of such tests may not be enough to reassure the public. Polling indicates that more than half of Americans believe the president has shown a noticeable decline in the past year, and his approval ratings remain low, hovering around the mid-30s in most surveys.
In the past year, Trump has visited Walter Reed on multiple occasions, including a CT scan to monitor potential cardiovascular issues. His health has long been a subject of debate, with even close allies like Health Secretary Robert F. Kennedy Jr. joking about his fondness for fast food and Diet Coke. As he approaches his 80th birthday in June, these concerns are growing.
A New Era of Leadership
Upon returning to the White House in 2025, Trump became the oldest person to assume the presidency, surpassing Joe Biden by a few months. Since then, his daily routine has taken a different path compared to Biden’s. Trump has maintained a high profile through frequent press availabilities, allowing him to project an image of energy and leadership despite his long-winded speeches that often leave others waiting awkwardly.
One notable example is his focus on the construction of a new White House ballroom. He has used surprise press events to highlight this project and often brings up the topic during televised Cabinet meetings, where his advisors frequently praise him. During an Oval Office event in late March, a discussion about California Governor Gavin Newsom turned into a commentary on Trump’s own mental sharpness, where he claimed that Newsom’s dyslexia was a sign of unintelligence.
Public Perception and Polling Data
A Reuters/Ipsos poll from April revealed that 51% of Americans believe Trump’s mental faculties have declined since 2025 and early 2026. The same poll found that only a quarter of Americans believe he is “even-tempered.”
In the same month, a Washington Post/Ipsos poll showed that just four in ten Americans now believe Trump has the mental sharpness required for the presidency, a seven-point drop from previous months. Additionally, concerns about his physical health are on the rise.
Political Responses and Trust Issues
These concerns have prompted Democrats to call for action under the 25th Amendment, following Trump’s controversial statements about Iran. His remarks, which suggested ending Iranian “civilization,” were seen as erratic and increasingly frustrated, especially as the conflict has dragged on longer than expected.
Compounding the issue is the lack of trust in the White House physician’s office. Former White House doctor Jeffrey Kuhlman, who served before Trump’s first term, expressed skepticism about the transparency of presidential evaluations. “After a decade of delusion, deceit, denial or delay from the administrations and White House physicians regarding presidential evaluations, my expectation bar is pretty low,” he told the Washington Post.
Another cardiologist, who previously served Dick Cheney, added: “This White House just doesn’t seem to want to acknowledge any physical ailment, but older people develop medical issues, and the president is almost 80 years old.”
